MYSA requires all Coaches, asst. Coaches, club adminisitrators and other official positions to fill out and submit a KidSafe form. Before you will be allowed to coach or hold a postion within SMSC you will need to fill out and submit the form online at MYSA.

If you are going to be holding more than one postion, for example, coach on a U-10 team and asst. coach on a U-8 team you only need to fill out one form.
